Eligibility
A Sri Lankan citizen by descent or by registration can apply for a Sri Lankan Passport.
Ordinary passports are valid for 10 years from the date of issuance except endorsement. A new passport should be obtained prior to the expiry of 10 years from the date of issue, or if there are no pages for visa endorsements.
Applicants are advised to apply for the new passports 2 and half years before expiry of existing passport with all required documents below, as the Kuwaiti Government requires 2 years validity to endorse visa.
Documents Required:
Issuance of New Passport in lieu of a current / expired passport
- Duly completed application form (Form ‘K’- 35 A). Fill the application using English Block Letters – (Download Application Here).
- Original Previous Sri Lankan Passport
- Three clear photographs of 3.5 x 4.5 cm in size.
- Please attach clear photographs with the full front face, both ears visible, without covering the face by the hair and without spectacles. Photographs should be taken within 03 months. Should not be gloss or semi-gloss.
- Original Birth Certificate (translations not accepted).
- A Copy of Sri Lanka National Identity Card (N.I.C) or driving license.
- Original Marriage Certificate (only for female applicants if the surname of the husband should be included in the passport: Optional).
- Academic and Employment certificates (if the profession is needed to be included in the passport: optional).
Fees 48 KD
If the applicant is a child under 16 years:
- Duly completed application form (Form ‘K’- 35 A). Fill the application using English Block Letters – (Download Application Here).
- A written request signed by both parents indicating that both of them do not have any objection to issuing an individual passport to the child. (Signatures of both the parents should be included as same as in the passport).
- 03 clear photographs of 3.5 x 4.5 cm in size.
Please attach clear photographs with the full front face, both ears visible and without spectacles. Photographs should be taken within 03 months. Should not be gloss or semi-gloss.
- Documents needed of the parents:
- Originals of the Parents Passports together with 02 photocopies of each of the data page of the passports with alteration page if applicable.
- Resident Permit Card or letter issued by the Immigration Authority of the Country of Residence indicating the status and category of parent’s visa.
- Original Birth Certificate of the child (translations not accepted).
- Citizenship certificate of the child or a copy of the payment receipt if the certificate is not issued at the time of application.
- Other Documents
- If parents were separated, documents indicating legal guardianship of the child.
- If one parent deceased, the death certificate of such parents.
- If a legal guardian is applying, proof documentations regarding guardianship.
Apply for a new passport in lieu of lost passport
If your passport is lost or stolen, it is your responsibility to inform the concerned authorities’ immediately. Failure to do so may result in your lost or stolen passports being used for illegal activities and you may be liable for aiding and abetting an offence.
IF YOU ARE IN SRI LANKA
- If your passport is stolen or lost in Sri Lanka, you can report the incident by phone to immigration Department hotline 0094 11 532 9326 and fax 009411-2885358. Operators are available 8.30 a.m. to 4.00 p.m. Sri Lanka time, Monday to Friday, excluding Government holidays.
- Make a complaint to your local police station as soon as possible.
- Police will transmit the details of your complaint to the Department of Immigration and Emigration.
- Upon receipt by Immigration the details from the police, Immigration and Emigration Department will take necessary measures to cancel the stolen, lost or missing passport.
- Passport reported lost or stolen can no longer be used for travel. The details of cancellation will be automatically shared worldwide to avoid misuse. Never use the lost or stolen passport again, even you find it later, after making a complaint. You are strongly advised to obtain a new passport immediately to avoid inconvenience
IF YOU ARE IN OVERSEAS
- Make a complaint to your local police station, then go to the CID and obtain a Police report.
- Download a stolen or lost passport complaint form. (Click Here for Application)
- Submit the duly filled complaint form together with Police report to the Embassy of Sri Lanka.
- Embassy of Sri Lanka shall transmit to the Immigration Department in Sri Lanka the details of the stolen or lost passports. Upon receipt of such information, Department will take necessary measures to cancel the stolen, lost or missing passport.
- Passport reported lost or stolen can no longer be used for travel. The details of cancellation will be automatically shared worldwide to avoid misuse. Never use the lost or stolen passport again, even you find it later, after making a complaint. You are strongly advised to obtain a new passport immediately to avoid inconvenience.

Without a copy – 144 KD
A Sri Lankan passport holder may apply for amendments of the data included in his/her passport and if the request is accepted by the Department, all such amendments will be included in the observation and alterations page. (Download Application Form here)

- Original Sri Lankan Passport
- Documentary evidence to prove the accuracy of the alteration or amendment

Per Alteration/ Endorsement – 4KD
All Country Amendment – 20 KD
Citizenship Registration – (within 1 year 15 KD, for late registration, 2 KD per year)
IMPORTATION OF Animal

Before bringing any live animal into Sri Lanka, prior authorization must be obtained from the Department of Animal Production and Health (DAPH).

The application form can be downloaded by visiting their website – www.daph.gov.lk. A duly filled application along with the vaccination record for the pet should be faxed/ emailed or submitted online to the Director General of the DAPH. Permission to import will be granted within a few weeks.
